PEAK 2.0Mission Statement: To be engaged resource partners with nursing homes in Kansas that hope to be rewarded for their pursuit of person-centered care and quality of life for residents and staff.
Improving the quality of life for those who live and work in long-term care settings across the state of Kansas is the primary goal for the PEAK (Providing Excellent Alternatives for Kansas Nursing Homes) program administered by the Kansas Department of Aging and Disability Services. Originally created about 10 years ago the PEAK program has been overhauled and the updated version was unveiled in 2012. In addition to recognition for homes that have achieved a minimum level of achievement in person-centered care, PEAK 2.0 also incorporates a tiered financial incentive package to encourage homes to begin and/or continue to pursue this model of care. The program seeks to encourage efforts in four primary domains resident choice, home environment, staff empowerment and meaningful life. To learn more about each domain click here. Within each domain core competencies and supporting practices to achieve these competencies are identified. PEAK 2.0 is open to all long-term care providers in the state of Kansas and currently has about 140 homes enrolled.
Kansas State University’s Center on Aging has a long history with PEAK. The Center was responsible for the development of culture change educational modules (scroll down the page) as part of the original PEAK project. Most recently the Center on Aging was awarded a contract in September 2012 to oversee the evaluation of PEAK action plans, provide educational support to providers in the program and evaluate the effectiveness of PEAK 2.0. As part of this contract the PEAK 2.0 project team at Kansas State University has created a variety of resources to help participating providers through all aspects of the process.
Planning Tools: The Unintended Consequences Mapping Activity and some Core Area Consideration Sheets will help you think about the impact of change. While writing an action plan, all people, departments, systems, tasks etc. need to be considered that are inevitably going to be affected by making major changes in the routine of your building.
Resource Videos: Please view the PEAK 2.0 Resource YouTube channel. This channel provides additional guidance on writing a good action plan and culture change. Please save this URL as the channel can be an ongoing resource and will be updated regularly.
Action Plan Template: The Action Plan Template should simplify the process of writing an action plan. Use the template to make sure that you aren’t missing any components to your plan (e.g. responsible person(s) and target dates). There is also a Microsoft Word version of this document.
Sample Action Plan: Please see the Sample Action Plan to view a well-written plan. This plan is specifically writing for “Food”. Notice that each component to the action plan is addressed. An action plan is a working document.
